Transition planning
Social work services for young people leaving school
Moving from school into adulthood can be both exciting and daunting for everyone involved.
You may need additional help and support from social work services as you move into your adult life, as well as making decisions about your next steps into further education or employment.
Transition planning meetings
These are arranged by your school, starting when you are around 14 years old to give plenty of time to think about options and to prepare.
The planning will focus on you and your views, hopes and ambitions will be taken into account.
This may involve a range of professionals who will meet with you and your family.
What will happen?
A social worker from the transition team will be invited to your meetings and will spend time getting to know you, your strengths and support needs.
They will make sure that you have the right support available to you in time for leaving school.
If you would like to know more about the support offered by the transition planning team, you can either ask your guidance teacher or contact the social work transition team.
